Incentive Plan Details
Delta Dental pays 70% of the PPO contract allowance for covered diagnostic, preventive, basic, and cast/crown services during the first year of eligibility. The coinsurance percentage increases by 10% each year (to a maximum of 100%) for each enrollee who visits the dentist at least once during the year. If an enrollee does not use the plan during the calendar year, the percentage remains at the level attained the previous year. If an enrollee becomes ineligible and later regains eligibility, the percentage drops back to 70%.

TELUS Health counselors listen and take action toward finding solutions — including up to three face-to-face mental health visits, negotiating health insurance benefits, or referrals to community resources for legal and financial services.

Claimant Assist Services
The Claimant Assist program offers special services to Long Term Disability claimants or Life Insurance beneficiaries at no charge. If you have Disability insurance coverage through NIS, Long Term Disability Claimant Services are available to guide and counsel claimants and their immediate family.

Coverage at a Glance
- Basic Life & AD&D (you): $50,000 — District paid, no reduction
- Supplemental Life (you): Increments of $5,000 up to $525,000 (not to exceed 5× annual salary). Reduces to 65% at age 65, 50% at age 70.
- Dependent Spouse Supplemental Life: Increments of $2,500 up to $250,000 (not to exceed 50% of your Supplemental Life)
- Dependent Child Supplemental Life: Options of $2,500, $5,000, or $10,000 (not to exceed 100% of your Supplemental Life). Terminates at child’s marriage or age 26.
