2026 Registration Process
Step 1: Complete the Math Academy Petition Form – Parent Consent Required
Fill out an electronic Math Academy petition for your school district or the school you wish to attend. Our Board Procedure 5011 requires a parental or guardian’s written consent for a minor seeking to be enrolled in a community college class. A parent or guardian will need to create an account to provide written consent. This process will take less than five minutes.
Ensure all required fields are completed accurately.
Math Academy Petition Digital Forms:
- Visit “Dynamic Forms“, click “Create New Account” and fill out all required information.
- An activation email will be sent to the email provided. Open the email and click “Activate your account”.
- Please select the school district the student will be attending during the Summer.

Step 2: Receive Math Academy Enrollment Confirmation
After submitting your Math Academy petition, you’ll receive an email from the Math Academy administration confirming your enrollment providing class details before the first day of instruction. We will make every effort to enroll students in the desired location and class; however, admission is subject to class and seat availability.
Step 3: Attend Your Summer Classes
Maximize your learning by attending all scheduled classes. If you must miss a session, inform your instructor in advance and arrange to make up any missed coursework.
