I understand that in order to maintain my eligibility in EOP&S, I must adhere to the following:

  • Upon acceptance to EOP&S, I am required to:
    1. Attend an EOP&S Orientation
    2. Submit the Mutual Responsibility Contract
    3. Complete an Educational Plan with an EOP&S Counselor

  • To maintain my active status in EOP&S/CARE each semester I must:
    1. Complete three (3) EOP&S counselor contacts per semester
    2. Apply for FAFSA or CA Dream Act and qualify for EOP&S Financial Aid eligibility
    3. Complete 12 units or more each Fall and Spring semester (unless approved for reduced units by EOP&S counselor) with a semester GPA of 2.0 or above
    4. Maintain satisfactory academic progress towards a certificate, associate degree and/or transfer goal
    5. Meet with an EOP&S Counselor at the beginning of the semester if placed on academic probation (GPA is below a 2.0 and/or complete less than six units). Two consecutive semesters on probation may result in dismissal from the program
    6. Report any changes in my Educational Plan to an EOP&S Counselor (for example: dropping or adding a class, change of major, etc.)
    7. Notify the EOP&S, Admissions/Records and Financial Aid Offices of any changes to my address and/or phone number

  • Upon completion of 70 units of college level coursework, eligibility for EOP&S/CARE services will be discontinued, UNLESS I am enrolled in an approved High Unit Major.

EOP&S agrees to provide services to assist me in meeting my educational goals which may include:
Academic, Transfer, Career and Personal Counseling Priority Registration
Academic Probation Intervention & Monitoring Grants*
Book Service* Transfer/Financial Aid Assistance
Graduation Cap & Gown Meal Vouchers
Educational Supplies Referral Services to Campus & Community Services
*Services are based on student’s Financial Aid Unmet Need and will be reported to the Financial Aid Office.

I understand that failure to comply with the EOP&S Mutual Responsibility Contract may result in Program dismissal. If dismissed, I must sit-out for one semester before reapplying to EOP&S and Director has final decision of re-acceptance.
