Each semester (fall, spring and summer) you must complete a new intake appointment to keep your information up-to-date. The appointment includes an Intake Form, Needs Assessment, and Student Contract.

Monthly Attendance Reports (GN6365)
For students that need assistance in filling out their Monthly Attendance Reports, please review the PDF below for general instructions.
You must submit a new Daily Schedule each semester and also if you change your schedule. This is used to verify your GAIN participation hours, to complete your MAR (Monthly Attendance Report), and to complete your Training Verification form for childcare.

Use this as a guideline for Student fees, the purchase price of some common school supplies and the request of books. You must provide your worker with original receipts. GAIN will not pay for excessively priced supplies or any supplies unrelated to the courses for which you are registered.

WIP form for Spring 2019
Halfway through the semester, have all your professors complete the Work in Progress form to let us know how well you are doing in your classes. Without this, we may not be able to complete your Progress Report for GAIN.
